Indiana Complex Care Coordination Collaborative - Preventive Care for People with Intellectual/Developmental Disabilities
Indiana Complex Care Coordination Collaborative (IC4) at IUSM is partnering with Special Olympics Indiana and the Village of Merici on the development of a person-centered website, “4ourhealth.org”. The website includes multimedia tools to teach patients with intellectual disability about self-management, health system navigation, and preventive care. This proposal will assess user experience engaging with the website and patient-reported outcomes as pilot data for a PCORI comparative effectiveness grant.
