Medication reconciliation technology to improve transitional care
Medication reconciliation is the process of gathering and verifying medication information, resolving discrepancies, making clinical decisions related to medications, and communicating information about medications to patients. Leading an interdisciplinary team of clinicians and scientists with expertise in medicine, biomedical informatics, sociology, statistics, and other fields, I have developed, implemented, and studied a computer-based tool to improve medication reconciliation by clinicians. This process can improve the quality and safety of care for patients, such as those being discharged from hospital to home. Two IUPUI institutions--Wishard Hospital and the Richard L. Roudebush Veterans Affairs Medical Center--are now incorporating aspects of this work into their processes of care to facilitate medication reconciliation.
